The following PDF forms need to completed and submitted prior to the start of rehearsals:

• Participation Fee
• Audition Form
• Parent-Dancer Agreement 
• Liability Release Form  
• Volunteer Form

The following PDF forms may be downloaded as needed:

• Leave of Absence Form – if you cannot attend rehearsals or performances
• Calendar – rehearsal, performance and deadline information
• Performance Etiquette – what is expected at performances
• Ad Sales Order Form – business advertising in SYPG program books
• Logo Wear Order Form – purchase of SYPG logo wear

Rehearsals are held at Springfield Dance, 5531 International Parkway, Springfield, IL 62711.
